Employment physicals

We can perform your employment and DOT physical.

Pre-employment physical examinations, periodically required medical examinations such as DOT, fire department physicals, and life insurance examinations are all provided at our office.

Please bring any necessary paperwork with you at the time of your visit.

School & sports physicals

We provide school and sports physicals for children, adolescents and college level students.

For the student who needs periodic school or sports pre-participation exams, we provide necessary exams and required diagnostic evaluations in the office. Please bring the appropriate forms to the visit.

Some insurances do not cover sports or camp physicals so a separate payment may be required at the time of service. To verify your benefits please contact your insurance company.
